The Consent for Treatment form is REQUIRED in order for you to
receive services and must be signed either before or on the day you
come in.  Your signature also verifies that you understand the
Policies & Procedures.

Instructions

1.  These foms cannot be filled in online. 

2.  Please download, print, complete and return to me via fax at 770-825-9066 at least one day prior to your appointment.

3.  If a fax is not available, bring them at your scheduled appointment or if you live nearby, you may bring them by the office from 9 - 5 Monday through Friday and leave them with Dr. Plaster's front desk receptionist.  She will be sure that I receive them.
