Services may be covered in full or in part by your
health insurance or employee benefit plan.
Please check your coverage carefully by asking the following
questions:
- Do I have mental health insurance
benefits?
- What is my deductible and has it been
met?
- How many sessions per year does my health
insurance cover?
- What is the coverage amount per therapy
session?
- Is approval required from my primary care
physician?
- Is pre-authorization required before my first
visit.
While I will do
everything possible to help provide you with information about your insurance benefits, it is ultimately
your responsibility to manage your health care benefits. Please bring
a copy of a valid photo ID and your insurance cards to your first visit.
Rates
Remember that the rates
below are not necessarily what you pay at your visit! If I am contracted with your insurance, you will pay
your co-pay (if you have one) or possibly full fee if you have not met your deductible. For example, if
you have met your deductible, your co-pay is $30.00, then you will pay $30.00 even though my fee is higher,
because once insurance pays the "negotiated amount" to me, your balance may then be written off. This
is NOT the case with "no show" or "late cancellation" charges. You are responsible for those charges
"in full."
- $170* for Clinical Interview and Initial
Assessment.
- $110* per 45-minute Individual Psychotherapy
Session.
- $110* Family and Couples Psychotherapy per
Session.
*These are my standard rates, but our contracted rates with your
insurance may vary. If you have insurance, then please refer to your
insurance plan to understand what your individual co-pay or out of pocket expense will
be.
Cancellation
Policy
If you do not show up for your scheduled
therapy appointment, and you have not notified us at least 24 hours in advance, you may be required to pay the full
fee for the cost of the session.
If you are a new patient who fails to show or
cancels at the last minute twice, you may not be offered another
appointment.
Your signature for the "Policies and Procedures" paperwork means that you understand the above information.
